Current and prospective co-op students should complete this course-planning document to ensure their academic courses and co-op rotations are on track. This form is intended for co-op students; however, any student interested in planning long-term academic and professional development may use it.

When

This form only needs to be submitted to co-op advisors when you have accepted a co-op position. Please ask your academic advisor to approve your academic courses beforehand.
